AND EARNINGS computed for EMPLOYEES during the Month.

It also lists the TAX AMOUNT and LIMITED EARNINGS (for those States that have Earnings
Cutoffs).

The Report SEQUENCE is by STATE, then by WORKMAN'S COMP CODE,
then Totals by EMPLOYEE in Employee Code Sequence.

Next to the Employee Name are listed across the page totals for:

         Regular Hours and Earnings

         Time and a Half Hours and Earnings

         Double Time Hours and Earnings

         Total Hours and Earnings

         Workman's Comp TAX

         Limited Earnings (Taxable Earnings in States that have EARNINGS LIMITS).

There are SUBTOTALS for each WORKMAN'S COMP CODE, for each STATE, and a GRAND
TOTAL for the Company on a separate page.
