This MONTHLY STATE INCOME TAX REPORT is a detailed breakdown of all
STATE INCOME TAXES DEDUCTED FROM EMPLOYEE'S Pay during the Month.

The Report SEQUENCE is by STATE, with each EMPLOYEE listed in Employee
Code Sequence, with Totals of EARNINGS and TAXES for the Reporting Period.

After a STATE Heading, the Employees are listed with
SOCIAL SECURITY NUMBER, EMPLOYEE NAME and CITY OF RESIDENCE, followed by:

         Current Earnings                Current Tax                

There are SUBTOTALS for each STATE and a GRAND TOTAL for the Company on a
separate page.
